Report card, attestation of studies and declaration of eligibility for instruction in English
Parents of a student and adult students who have attended an establishment of the Centre de services scolaire du Littoral (CSSL) can obtain:
- Copy of a report card;
- Attestation of studies;
- Declaration of eligibility for instruction in English.
How to make a request:
Print, complete and sign the application form
- For students currently attending or who left a CSSL school less than three years ago: send the application to the secretariat of the school last attended.
- For students who left the CSSL more than three years ago: send the request to the General Secretariat.
Two possible options to obtain the document
Option A
Pick up the document at the CSSL: simply indicate this on the request form and send it by e-mail or regular mail. The CSSL will contact the applicant within five to ten days to indicate when the documents will be available at the address indicated at the bottom of this page. Identification (ID) is required to pick up the document ordered.
Option B
Receive the document by mail or e-mail: send the application form along with a photocopy of your ID (with signature). The CSSL will send the document within five to ten days, and the photocopy of the ID will be confidentially destroyed as soon as the request has been processed.

The CSSL offices are open Monday to Thursday, 8 a.m. to 12 p.m. and 1 p.m. to 4:30 p.m., and Friday, 8 a.m. to 12 p.m. During the summer months, it is advisable to contact the CSSL before visiting, as the schedule is different. Do not show up without first obtaining confirmation that your documents are ready.
